  • A thorough screening process of medical conditions, medical history, injury questions and lifestyle factors, diagnosis, treatment information, health related information. This information is collected and stored for the purpose of health diagnosis and treatment forms part of our contract to provide our services. In addition, we will only examine and treat you with your full consent.
  • If you make enquiries via the website or by email, social media we will only contact you with your full consent.

We only share your personal information with your full consent. For example, we need to contact a medical practitioner as part of your care.

 Retaining your personal information:

Whilst receiving treatment from our clinic we will continue to store and use your personal information. Once you have been discharged or no longer receive treatment with Back on Track, we are required to retain your personal information for a minimum of 8 years from your last contact with us or until you are 25 (26 if you are 17 when treatment ends).

 Your rights:

As we process your personal information, you have certain rights. These are rights of access, rectification, a right of erasure and a right to restrict processing.

Cancellation Policy

In order to respect the time of the Sports Therapists and to offer availability to all our patients, we kindly ask for 24 hours’ notice to cancel an appointment. In the absence of 24 hours’ notice, or in case of a ‘no show’ or late arrival, the full fee for the scheduled service will be payable.

Terms and conditions

Payment terms:

Our standard payment term is that payment is taken at the time of treatment by the means of cash or via the payment terminal provided.

Unforeseen circumstances:

On occasion, unforeseen circumstances may require us to cancel a booking. In such circumstances you will be given as much notice as possible.

Location of services:

Currently, we provide our services at your residence/address. We have the right to change the location if necessary.

Therapists:

Anna Thomson is the Owner and Partner of Back on Track. Tara-Jayne Hewitt is Owner and Partner of Back on Track. The therapists maintain adequate professional indemnity insurance and are qualified to provide the therapy services.
